As an Independent Social Worker I undertake Safeguarding Adult Reviews (SARs). These were first introduced by the Care Act 2014, and usually take place when there has been a death or serious injury to an adult with care and support needs. They are separate to any investigation and provide in-depth analysis and critical reflection of events as they were at the time with the aim of positively affecting future practice and systems.

'Peter' focusses on the complex situation surrounding the death of a man who experienced long-term issues around mental health, drugs and alcohol and homelessness - peter-ph-sar-v6-final.pdf (gloucestershire.gov.uk)
'Stanley' explores self-neglect masking 'mate crime' in the form of financial abuse from a neighbour. 'Stanley' was elderly with a long history of being engaged with mental health services. https://safeguarding.dudley.gov.uk/media/20045/sar-stanley-published-12052022.pdf
Mentoring, coaching and supervision can be one-to-one or as part of a staff group/team. My approach is centred on support, encouragement and guidance around critical reflection of self and practice as well as goal setting with the aim of encouraging positive growth and development as a professional.
Consultancy work can include working with organisations to review safeguarding adult policy and procedures.
